Case Manager Job Description Template

We are searching for a Case Manager who will oversee the collaborative process among patients, health care delivery systems, support systems, and financial reimbursement sources with the goal of helping patients effectively manage their health care. Your role will include developing, following, and evaluating treatment plans for a broad range of patients. 

The ideal candidate will have a strong knowledge of budgeting and patient rights policies, and possess the ability to work with other health and human service workers to facilitate patient progress. A diverse portfolio of cases and case management certification is a plus. 

Case Manager Responsibilities:

  • Manage a diverse range of cases
  • Provide care in an efficient, timely, and patient-centered manner
  • Create service plans and review progress
  • Construct admissions procedures so all patients receive equal quality care in a timely manner
  • Coordinate referrals to other health and human services providers
  • Accurately record case reports
  • Provide cost-effective treatment plans
  • Oversee intake and discharge actions

Case Manager Requirements:

  • Experience as a Case Manager and devising treatment plans
  • Background in health care, social work, or a similar field
  • Strong knowledge of applicable legislative rules and guidelines
  • Excellent communication and interpersonal skills
  • Outstanding problem solving and multitasking abilities
  • Bachelor’s degree or Master’s degree in social work, communications, health care, or related field
  • Bilingual language skills preferred
